Key personal details that are regulated for the provision of your Practising Certificate or Membership can be updated through the QLS Records & Member Services team. Enter details of changes to your name, or request PC restricted supervision conditions be removed from your PC.

You can submit the following requests to the QLS Records & Member Services team:

• Change in Name
• Remove / Reduct Supervision Practice Period (Coming Soon)
• Request Refund (Coming Soon)

• Update Employment (Queensland Practising Certificate holders only)

Changes submitted below will be reviewed and you will receive an email once approved.

Please note during the renewal period you will not be able to proceed with your renewal application until the change has been updated by QLS. Due to high volumes of enquiries during this time delays may be experienced.
